Q: does it come with the gpio connector (header) attached

No - very disappointed by this myself as GPIO is necessary for most of the use cases for this hat. It makes this hat basically useless, sadly.

Q: Confused: i bought this and unsure what i am plugging the micro usb ports into? thanks

On the Pi Zero, you plug the little circuit board between the data micro usb connector on the Pi zero and the similar connector on the Hub Hat immediately above it. On the full size Raspberry pi, you use the included cable to connect between one of the Pi USB connectors and the micro usb connector on the Hub Hat.
